Cefuroxime axetil is a second-generation cephalosporin antibiotic and the orally active prodrug of cefuroxime. Its structure features a 7-aminocephalosporanic acid core with a methoxyimino-furyl side chain at C-7 and an acetoxyethyl ester at C-3. This ester modification increases lipophilicity and allows efficient absorption from the gastrointestinal tract, after which nonspecific esterases in the intestinal mucosa and blood rapidly hydrolyze the ester to release the active cefuroxime.
It is supplied as a white to off-white crystalline powder, practically insoluble in water but freely soluble in acetone and methanol. The compound is manufactured under cGMP conditions and is available in different polymorphic forms, with Form I and Form II being the most common. As a broad-spectrum antibiotic, cefuroxime axetil is widely used in the treatment of respiratory tract infections, urinary tract infections, skin and soft tissue infections, and Lyme disease. It is also a key intermediate in the synthesis of other cephalosporin derivatives. The material is typically formulated into film-coated tablets or oral suspensions for human use, and into veterinary products for companion animals.
The molecule contains a beta-lactam ring fused to a dihydrothiazine ring, with a methoxyimino group that enhances stability against beta-lactamases. The acetoxyethyl ester at the C-3 position not only improves oral absorption but also serves as a prodrug moiety. The furan ring contributes to the overall lipophilicity. Cefuroxime axetil is sensitive to moisture, heat, and light; it should be stored in tightly closed containers under cool, dry conditions. The powder exhibits polymorphism, and the amorphous form is generally preferred for solid dosage forms due to its better dissolution profile.
