Iohexol is a non-ionic, monomeric, water-soluble iodinated contrast agent. Its chemical structure consists of a 1,3,5-triiodinated benzene ring bearing three hydrophilic side chains: two 2,3-dihydroxypropyl groups and one N-(2,3-dihydroxypropyl)acetamido group. This symmetrical design yields a low osmolality (approximately 844 mOsm/kg H2O at 300 mg I/mL) and high hydrophilicity, which together reduce the risk of adverse reactions compared to ionic contrast media.

First synthesized in the late 1970s, iohexol is produced through a multi-step process starting from 5-nitroisophthalic acid, involving iodination, amidation, and catalytic hydrogenation. It is supplied as a white to off-white crystalline powder, typically with an assay of ≥98.5% (HPLC). The compound is freely soluble in water, allowing the preparation of injectable solutions at concentrations up to 350 mg I/mL. Common grades include pharmacopeial (USP/EP) and technical grades for research use.

As a diagnostic agent, iohexol is widely used in computed tomography (CT), angiography, urography, and myelography. Its low osmolality and non-ionic nature make it suitable for patients with compromised renal function or a history of contrast medium reactions. In the pharmaceutical industry, iohexol is also employed as a reference standard and in the development of novel contrast agents.

The molecule’s three iodine atoms (each contributing high X-ray attenuation) are shielded by hydrophilic groups, enhancing water solubility and reducing protein binding. The absence of charged groups in solution minimizes osmotic stress on blood vessels and red blood cells. Iohexol exhibits excellent chemical stability under normal storage conditions, with a shelf life of up to 5 years when protected from light and moisture.
